Insurance & Payment Responsibility Notice

Please remember that your insurance policy is a contract between you and your insurance company. While our office may verify benefits for speech/language evaluation and therapy services, all benefit information we provide is strictly an estimate and is not a guarantee of coverage or payment.

It is the patient’s/responsible party’s responsibility to fully understand their insurance benefits, including deductibles, copays, coinsurance, visit limits, and authorization requirements, to avoid unexpected charges.

Many plans only cover services after the deductible has been met. If you have a high-deductible plan, you may be required to pay out of pocket until your deductible is satisfied. Some plans also limit the number of therapy visits allowed per calendar year.

Any portion of services not covered by insurance will be billed directly to the responsible party, and payment is ultimately the patient’s responsibility regardless of insurance determination.
